Medicare Limited Coverage Determination Policy Diagnosis Codes
1. Choose the national or Indiana limited coverage determination procedure using the drop down box
2. Type the patient's diagnosis (all digits, including decimal) in the box (optional)
3. Click on "Search"
4. A description will appear for all covered diagnoses unless otherwise indicated as "not covered".
